Non-adherence is the largest preventable crisis in chronic disease

An estimated 125,000 deaths and $300 billion in avoidable healthcare costs occur annually in the United States alone — not from treatment failure, but from patients not taking their medications as prescribed.

Between 30% and 50% of patients with chronic conditions do not adhere to their prescribed regimens. Yet most interventions treat all non-adherence the same — a critical error with measurable consequences.

Scala Carta Foundation exists to change that by funding and disseminating the research infrastructure that makes precise, behavioral phenotyping of non-adherence possible at scale.

MAP (Multidimensional Adherence Parameters) is the validated instrument that distinguishes INA from UNA, generating PEACS phenotypes that drive targeted intervention. Independently authored by Philip Morisky and deployed through ATLAS, MAP is the shared measurement standard that TESSERA GRC is built around.

TESSERA GRC establishes the governance, risk, and compliance framework for how MAP data is held, attributed, and shared across jurisdictions including the EU (GDPR), UAE (PDPL), and US (HIPAA). Institutions joining TESSERA inherit this framework — they do not have to build it themselves.

A no-login, browser-based kiosk tool for pharmacies, mobile outreach programs, and telehealth services to administer MAP assessments outside of clinical settings. The inaugural TESSERA Summit will convene member institutions, researchers, and industry partners for the first global conference dedicated exclusively to behavioral adherence phenotyping science. One agenda. One network. Every major contributor to this field in the same room.

Science must travel where the burden is highest

The global burden of medication non-adherence falls disproportionately on low- and middle-income countries, where chronic disease rates are rising fastest and research infrastructure is thinnest. TESSERA GRC was designed with this asymmetry in mind.

Through our LMIC Grant Pathway and Research Contribution License (RCL), researchers at qualifying institutions can access full TESSERA membership, including MAP instrument access and co-authorship on normative studies, at no cost.

Donor support is what makes this possible. Every contribution to Scala Carta Foundation directly funds the participation of a researcher who would otherwise be excluded from the global evidence network.

What happens to
your data inside TESSERA

Every institution's cohort is isolated. Your patient records never mix with another member's data. What contributes to the global normative dataset is anonymized aggregate MAP output — phenotype distributions and domain scores, stripped of all identifiers.

The TESSERA Data Governance Charter defines how MAP data is attributed, retained, and shared across jurisdictions. Member institutions inherit this framework on entry — they do not rebuild it from scratch for each study.

TESSERA data processing for EU member institutions operates under a documented lawful basis. Data subject rights, retention limits, and processor agreements are covered in the Charter.

UAE · PDPL

Gulf-region members operate under the UAE Personal Data Protection Law framework. TESSERA maintains separate processing records for GCC jurisdictions.

US · HIPAA · IRB

US clinical and research sites using MAP through ATLAS receive an IRB-ready data dictionary and BAA template on request. De-identified aggregate outputs do not trigger HIPAA obligations.

Scala Carta Foundation issues your formal TESSERA membership confirmation, credential letter, TESSERA Study ID, data governance agreement, and — for tiers that include it — your ATLAS workspace key provisioned for your data residency region. LMIC scholarship recipients receive a fee waiver letter alongside their membership package.
